RN Case Manager Home Health
Eklego is seeking a RN Case Manager for our client's Home Health division.

This is a direct-hire, full-time position, working Day time hours.
The Registered Nurse Case Manager works independently throughout Greensburg, PA, providing initial and ongoing assessment of client's skilled nursing needs.
The RN Case Manager will also implement care plans and make referrals to other services that support the patient's care plan.
You'll also be provided with all the supplies you'll need for documentation and treatment.
Requires:
Current RN license for Pennsylvania Valid Driver's License and reliable vehicle Prior community based experience in a Medicare-certified home health and/or hospice/palliative agency preferred.
